1. The need for a health advocate;

2. Hospice vs. No Hospice;

3. Where do you prefer to die, at home or at the hospital; 

4. How do you prefer to die, with heroic interventions or naturally;

5. What wishes do you want carried out; and, 
6. What are you doing now to prevent your premature death?
